Note: Users will be directed to this form during the advertiser verification process if necessary. This form is to be used by Google Ad Grants accounts and agencies that are verifying the identity of their client.

To increase transparency for users, we require agencies to verify the identity of the client (i.e. the end advertiser) they represent and/or provide additional information regarding their name and location. For most ad formats, the information provided below will be displayed in an automatically generated disclosure. Learn more in the advertiser verification policy.

Before completing this form, please ensure that your company or agency owns the Payments profile for this Google Ads account and that the "Account type" is set to "Organization" in the billing setup.
