OK Google detection not working. AGAIN!!!!
Man, what a piece of garbage Google software is! How else?! They are busy focusing on implementing DIVERSITY instead of making sure their gear works! Google has become the shittiest manufacturer and software developer out there!

Try changing language on BOTH phone and watch to English US. In most cases it work by changing settings on phone, google account and wear OS to English US.
Also, try to send the feedback directly to the WearOS team.
Using the Wear OS app on your phone, go to Menu > Help & feedback > Send feedback

just adding another data point that OK google isn't working (never worked) on my Ticwatch Pro 3 but assistant works otherwise (i'm in the US). Hopefully Michael Wechsler et al. get it sorted, the hands free activation is one thing I'm really looking forward to working, often find myself with my hands full and wanting to turn a light on or something.

How I solved:
Ticwatch E2 Wear OS watch is not detecting 'ok google' voice command
From several trials, I noticed that the problem came from the latest "Google" version pushed to the watch.
When the watch was in the original factory "Google" version, there was no problem. But after "idle time" (charging, standby mode, reset, etc.) the problem appeared. I noticed that the latest "Google" version was pushed to the watch during these "idle time".
The actions:
1. [on phone] Disconnect the watch from the phone "Wear OS"
2. [on phone] Delete the watch from the phone bluetooth memory
3. [on phone] Unistall the "Wear OS".
4. [on phone] Reinstall the latest "Wear OS"
5. [on watch] Do the "Factory reset" (go to "setting", go to "system", go to "disconect & reset")
6. [on phone & watch] Follow the whole fresh watch to handphone pairing steps.
7. [on watch] Immediately deactivate application "auto-update" feature on watch (go to "play store", scroll to the bottom to find "setting", turn off "apps auto update")
Now, the watch is using the factory tested, stable "Google" version (that can detect "ok google' voice command). And it will stay on this stable version, since we havae deactivate the "apps auto update" feature.
We can re-activate "apps auto update" after Google released the next stable "Google" version for the watch.

Unfortunately "OK Google" doesn't work on the watch. My Google Assistant language is set to English (US), same is the system language on the phone, the language on the watch. I wasted hours trying to set it up, resetting the watch several times, reinstalling Google Home app on the phone but nothing works.
On top of that there is no way to backup the watch before resetting so I have to install all the apps over and over again after every attempt. I'm done with that.
I am super disappointed by Wear OS so far. Makes the device almost useless. I'll definitely go for Samsung next time.

Same problem for "ok google" hot word hands-free detection here.
I have two watches, the HUAWEI watch and Ticwatch pro 3. Both watches have long-push for google assistance working fine. Phone and watches were set languages for US English.
For the Huawei watch, I uninstalled the update for Google in the watch's google play store, cleared the data of Google app and "ok google" detection works right away after I uninstall the Google app update. The working Version is 7.8.22.25.arm, however, this version is too old, although it can detect "ok google" hot word and recognize the question correctly, it will respond there is a problem and something...
For the Ticwatch Pro 3, since the watch was launched a month ago, the factory default Google app has version 11.30.9.25.arm, there is no way to install a lower version of the Google app, so no luck for the Tickwatch pro 3.
@Michael, So there must be something wrong with the new version of the Google app on the watch, nothing to do with the app on phone.
