I'm using a Pixel 5, and as the title states during calls every few seconds my headset will tell me it's been disconnected then reconnected. This issue only occurs during phonecalls and i'm using the Phone app. This issue does not occur using any media such as Spotify or YouTube and can get the entirety of my devices battery life without a disconnect. I have checked settings and permissions, I have unpaired and forgotten the headset to pair it again. I have restarted my phone and the headset itself, and didn't have this issue when using the same headset on my Pixel 3 that I have recently upgraded from. If information is needed, I use the Raycon Everyday Bluetooth headphones.

I am having the same exact issue with my pixel 5 and I am also using Raycon e25s. Did you find that resetting your phone worked?

Same issue here, but with Monster Clarity 102 Airlinks. Chatted with someone at Support.google yesterday who said I needed to either send the phone in for repair or take it to an approved repair shop...

I have the same issue with Plantronics Fit 3150 earphones and Nissan Pathfinder. Can listen to music without issue but phone calls keep disconnecting and reconnecting. Saw some posts about playing with developer options, but nothing has worked. Google next port of call!

I am also having a similar problem. 
On my Pixel XL everything worked properly. (Ear Fun Free BT buds).
On my Pixel 5 they work fine with YT music and stay connected for hours with no problems.
With Skype calls (over WiFi) they work for a few minutes, disconnect, then reconnect. This repeats, making the calls challenging. I have not tried Phone or other apps, as all of my business calls come in via Skype.
I set Skype to Not Optimized in Battery Optimization in case that was triggering something.

In addition to contacting Support, I suggest you use your phone to send feedback directly to Google's development team:
 
Go to Settings >  About phone >  now tap on "Send feedback about this device."  Type your feedback.
 
To include your system logs, check the "Include system logs" box. These logs help Google when you report a technical issue. Tap Send. Your message, system information, and email address (if you're signed in) will be sent to Google. 
 
Although you will not receive a reply unless additional input is needed, be assured that the development team does make use of the information they receive via this feedback mechanism.

im experiencing the same problem with my pixel 5 . this is happening since i got the phone last October and its so frustrating really. im using urbanista Stockholm plus wireless earphones.
during phone calls it will disconnect and then after few seconds it will pair and connect again and the the cycle   happen again and again and again.

Same issue for me with a Pixel 5 / Android 11 and CellularLine T12 Bluetooth headset. I am disconnected / reconnected 2 or 3 times every minute only during phone calls, I have no problem with music.
