Form needed: people signup for multiple dates, easily see who is signed up for each day

I am trying to make a form whereby team members can indicate which days of each month they are available for meetings. In the results sheet, I would like to easily be able to filter to display all team members that are available for any given day. So whereas the collection collects a list of dates per person, the output should be a list of all people who have selected any given day. 
I have tried using a checkbox grid labeled Mo-Sun in columns and week 1-4 in rows, as well as a simple checkbox question with each day of the month being one checkbox. However, both of these options result with all selected dates being listed in one cell divided by commas. If I then use the "separate data into columns" feature, it doesn't update correctly if someone changes their selection. 
Greateful for your help! Have spent way to long on this and cannot figure it out!

We would like to help you with this but because there are so many variables that can affect your results, we need to see a sample of your problem.  Would you please take a look at this post about Best Practices When Asking a Question and share a sheet that has the permissions set for “Anyone with link can EDIT”?  Thanks!

Either of these two outputs would work fine. 
The easiest way would be to have people mark the checkboxes on the bottom table themselves on days they are available. However, I'd like to be able to collect these responses confidentially, so every teammember doesn't see what days everyone else is available. That's why I'd like to collect it using a google form.

Thank you for the sample file but there does not seem to be any Form responses or a sheet for that in the file.  Would you please include sample data of what you are expecting to see so that any solutions I or another volunteer comes up with can be tested?  Thanks!

As you can see, it collates all available dates into one cell instead of collecting responses separately like i need them. I would like the final results to look more like the example I put into the "Ideal Results Format!" tab.

In your desired results you show names, not email addresses.  Where are the names and their associated email addresses located?
