I arrived at a form called “Apply for Google Wallet/Payments account reinstatement”
If you've arrived at this page, it means that your Google Wallet online account is currently suspended. Never fear - you can reinstate Google Wallet on your Google Apps account by filling out this short form. You’ll be required to verify your identity to reinstate your Wallet account by providing information about a recent order and by providing information about your payment method on file.
Why was my Google Wallet account suspended?
Your account was suspended as a precautionary measure to protect your payment information because Google detected that an administrator at your organisation initiated a password reset for your email address or you changed your own password. However, this suspension only affects your access to Google Wallet and you should still have access to your other Google Apps services such as Gmail, Calendar, and Sites.
How can I avoid this in the future?
You may wish to sign up for a new Google Wallet account using a personal email address, rather than your Google Apps account. Creating a new Wallet account using a personal email address will reduce future suspensions of your Google Wallet account for password changes. If you don't have an alternative email address, you can create a new email address with Gmail for free.
