Also want to point out that this doesn't just affect mobile data, but also voice and SMS. Our phones are reduced to 6 inch wifi tablets.

We have followed extensive steps through our carriers and Google support to check coverage, reset APNs, change SIMs, factory reset, revert to older builds, but still the issue persists. The "!" is not a temporary indicator such as seen when switching from wifi to data or when turning the phone on, or brief moments of loss such as other tickets describe, it continues for hours at a time and is only very briefly solved by restarting the device before the "!" returns.  The SIM status indicates the phone is receiving good signal strength, but the connection is somehow being continuously interrupted. 

I have also noted that the problematic signal seems to occur following a loss or lack of availability of LTE service. The "!" seems to always follow a period of 3G/H+ signal. When the phone is able to remain with an LTE signal, the loss of connection doesn't seem to occur.  However, the connection sometimes works briefly with 3G/H+ (that is, I'm able to use cellular service), it just is always a precursor to the connectivity loss for me.  

I have also noted that while the system is showing the "!" symbol, the phone cannot recognize SIM card changes until the phone is restarted. The "!" icon and SIM Status menu signal indicators do not change at all, as if frozen, when the SIM card is removed and replaced. Even with a SIM card lock, the phone does not prompt for the PIN when it is replaced and no message or icon indicating the SIM was removed shows.

LMY47D, T-Mobile. This issue started last night for me. Toggling airplane mode doesn't help. Rebooting sometimes fixes it but sometimes multiple reboots are needed.
 
This afternoon I got a "no sim" message until I rebooted.

Same issue here. T-Mobile LMY47M. Started occurring on May 4th as well. All symptoms are exactly as described in OP. Nothing seems to alleviate the issue.
