How can I get a list it all email addresses associated with my name

Try  Can't sign in to your Google Account , select "You forgot your username or the email address you use to sign in."

 

Of course you will need to be able to verify this information with Google;  for security reasons such information is not given out on demand.  If you can't verify the information, Google is unable to help.

 

If you can sign into an account, try going to https://security.google.com/settings/email , expand on "Advanced" as well.

 

For account names and passwords used to sign into third party apps or websites;  you need to ask those people; Google has no such records.  Or if possible, check for welcoming emails.  May also be saved in a Password Manager in your browser, or a Password app, if installed.
