EEOC and OFCCP form

This article is for Hire Admins.

Add a questionnaire to every application on your job site to gather applicant data for U.S. Equal Opportunity (EEOC) filings or OFCCP compliance. After enabling, all applicants will see the questionnaire when applying to a job that’s been published through Hire.

Enable EEOC and OFCCP compliance questions on job applications

  1. Sign in to Hire.
  2. On the left, select Admin and then Job applications.

  3. Turn on US EEOC/OFCCP form​.

  4. Select US EEOC/OFCCP form.

  5. To also include OFCCP questions, select Include OFCCP Questions.

  6. Scroll down to preview the questionnaire. If you included OFCCP questions, they’ll be placed after the EEOC questions in the form.

  7. Select Save.

Invite candidates to fill out the questionnaire

If you haven't collected EEOC and OFCCP compliance data from a candidate, you still have the option to request that information through a follow-up email.

  1.  Sign in to Hire.
  2. On the left, select Admin and then Candidates.
  3. Select a candidate’s name.

  4. On the right, select Show more.

  5. Select Request diversity information.

  6. Select a template and reply-to address.

  7. Select Send.

Assign a user compliance reporting permissions

Hire admins need to assign permissions to a user to review compliance data. By default, Hire admins are not given access to this information due to the sensitivity of this data, but can grant themselves permissions to review compliance reporting. After acquiring compliance reporting permissions, you'll be able to export compliance data into a CSV file or Google Sheet.

  1.  Sign in to Hire.
  2. On the left, select Admin and then Users.
  3. Select the user you’d like to assign compliance reporting permissions.

  4. At the top, select Actions and then Add role.

  5. Select Compliance reporting.

  6. Select Add.
