How do I add an electronic signature in a google form after one or multiple questions?

Hi Jon - I am not aware of a formal signature option but would something like
"By completing and submitting this form you agree to the terms and conditions.
A full copy of the T&C can be viewed at www.yourwebsiteaddressetc.com"

be suitable.
You then ask a YES / NO question and / or for the name in full.
You could also require the user to be in a Google Account and log the email address.
A web search for "How do I create an electronic signature in google forms" gives several You Tube videos to watch. You may find a possible solution there .

You can submit your concerns or product improvement ideas via Help > Report a problem (the question mark ? in the bottom right corner of the Form Designer).
The team reviews all feedback sent to them but is unable to respond to individual users. The more users who comment on or request a particular feature via the in-app feedback, the more likely it is that Google will make a change.
