Google Forms Picture Upload not working

I am trying to upload a picture onto google forms. I click the picture icon and the usual screen I get to browse the computer to select the picture is no longer there. When I click the picture icon a blank box pops up.

Please clear cookies etc and does the same occur with different browsers?

Same issue, and occurs no matter which browser or device I try to use.

Same issue - have cleared history and cookies and checked that pop-ups weren't being blocked

Same here. What I did find out is that if you enable Google Photo under Additional services in the admin console this will fix it.

Same. Worked fine this morning, now getting the blank screen upon attempt to upload image. (11/6/19, 2:15 PST) Tried in incognito mode in Chrome and in a different browser (Safari.)

I have a Google form that I want to send out tomorrow, and the background picture I want won't upload.
