Collecting respondents' email addresses

If you use Google Apps for Work or Education, you can record the email addresses of the people who fill out your form.

To do this, check the box next to 'Automatically collect respondent's [yourdomain].com username' when you create the form. Recipients of your survey will see a message at the top of the form explaining that their username will be collected automatically.

Require sign-in to view a form

As a Google Apps user, your form settings will require login to your domain by default to add a layer of security to sensitive files.

If you want to allow respondents from outside your domain to view your form, uncheck the box next to 'Require [yourdomain].com login to view this form' and click OK after reading the warning that appears.
