Some things to note before submitting this form:

1) Applications can be blocked when they are not compliant with Google's TOS or AUP.  Please make sure you read through both and are compliant.

2) One of the most common compliance issues we see is an attribution violation, which means:

Your application violates Section 3.2.2(b) of the terms, which requires developers to “display all attribution that (i) Google provides through the Services (including branding, logos, and copyright and trademark notices); or (ii) is specified in the Maps Service Specific Terms.” In addition, developers are not allowed to “modify, obscure, or delete such attribution.”

Please be sure your application displays “powered by Google” branding, the Google logo and/or Google copyright and trademark notices on the map, and that it is is clearly visible, as required by the Terms of Service.
