For example, in my back home routine, I added a custom response and a media action. It used to run perfectly, but it suddenly only executed the custom response, then showing the media action command on the screen and said "Sorry, I don't know how to help with that." The media command is a playlist on Spotify.

I've checked the media setting, it is linked. And if I say "Hey Google, play  (playlist name) on Spotify" it still works.

After I noticed this issue, I tested my other routines that contain media action, turned out they now all have the same issue, only the media part isn't working and keep telling me "Sorry, I don't know how to help with that." 
I was like, but you used to know how to help with that...

Does anyone have the same or similar experience before? Could someone let me know how to fix it?

Have you tried deleting the routines and then recreating them?
 
I have seen a few posts today about Spotify here in the Google Assistant Community as as the Nest Community. I will check with the Google Assistant team to see if there are any known issues.

Yes I did create some new routines to replace the old ones. But they still didn’t work.
I even downloaded Google Home and Google Assistant apps again and updated iOS. 
Nothing helps so far. Very frustrating.

I'm having exactly the same problem, I had a routine setup for my daughters bedtime and now all I get is "sorry I don't understand" when it comes to the playlist part

I have the same problem. I have a daily scheduled routine to play music from my speakers around the house and it suddenly stopped working around 1 month ago. I tried to delete all the routines, re-create them from scratch, change the device to play the routine to a different Nest mini. Also trying creating the routines from an iPhone to exclude app issues. Nothing seems to work at the moment
