Why is my Bluetooth headset no longer playing audio when I get a phone call?

I haven't needed to use my Bluetooth in quite a while so I don't know when the issue started, but currently I am highly frustrated with trying to solve it.

My settings are right, the headset is set to play media and phone calls, yet if I answer a phone call with them, no audio comes through the Bluetooth headset, but instead out the phone's speakers.

This is very annoying as it's the opposite of why I need to use a headset in the first place. So others don't hear the sound.

I've tried just about everything even restarting the phone and device like I've seen mentioned, and it doesn't work.

Media Audio plays fine until I get a call.

I have tested this on multiple Bluetooth devices with the same result.

Very irritating as it used to work flawlessly.
